It is marked "Yamaha" on the end where the mouthpiece gets inserted.  The serial number is 682703.  I do not know the model number. It measures 21 1/2" to the end of mouthpiece. Without the mouthpiece, it is 19 1/4" long. I don't know if it is silverplate or nickle plated, but I is silver in color and is somewhat tarnished.  The keys do move up and down freely and the slides go in and with relative ease.  The bell has a shallow dent that looks like it could be rolled out easily and there are a couple of tiny dings on curved pipe. I do not know what key it is, such as C, B flat, etc. The case is a hard rigid case and does smell a bit musty inside.  I don't know much about horns, so I am limited with respect to answering technical questions.
