I presume that this sheet music was a promotional premium that you sent for through Wrangler Jeans. The 1965 copyrighted music was attributed to Norman Meade and lyrics to Bob Elgin. The back page shows a seven sequence instructional on how to do the Shake. The sheet has a horizontal crease through the middle which was probably a mailing fold. It shows some use and wear but defects are minor. The sheet music originally probably was accompanied by a record. I found a website a while back, perhaps on You Tube which played the record done by I believe the artist Joyce Williams. The record  looked like one of those floppy vinyl promotional discs.
